Please be on time to your appointment. If you are more than 15 minutes late for your appointment, we will have to reschedule you. The time of your appointment starts when you check in with the receptionist.

Method of payment. We accept cash, check, Visa, Master Card and Discover.
List of current medications, including any over the counter medications and vitamins
Questions for your provider
Any records or documentation from previous medical care
If you schedule an appointment with our office and need to cancel please do so within 24 hours of the appointment.
Failure to do so will result in a $40 charge.
Refills should be called in by your pharmacy and we ask that you please give us 24-48 hours for routine prescriptions. Simply have your pharmacy fax us at (408) 996-3550. You may call your pharmacy to check the status of your refill. Routine prescription requests are not accepted after 5pm or on weekends.
Patients that need a written prescription for mail order, should speak to their provider's Medical Assistant.
Completing forms for camps, schools, employers, license renewals, extended care facilities, and others not mentioned. (If office visit is necessary to complete specific information on these forms, there will not be a charge for completing the form.)
Writing and dictating letters for an airline reimbursement in the event of illness, for employers, excuse from jury duty, selective surgery, or any other reasons for a letter.

These above services/charges are not billed to Insurance.
Writing of Triplicate's will be a minimal office visit charged to your Insurance if you have a copay that will be collected at the time the Triplicate is given.
