Please be on time to your appointment. If you are more than 15 minutes late for your appointment, we will have to reschedule you. The time of your appointment starts when you check in with the receptionist.

Method of payment. We accept cash, check, Visa, Master Card and Discover.
List of current medications, including any over the counter medications and vitamins
Questions for your provider
Any records or documentation from previous medical care
If you schedule an appointment with our office and need to cancel please do so within 24 hours of the appointment.
Failure to do so will result in a $40 charge.
We accept most major insurance plans. We accept all PPO's. HMO's must have a medical group of SCCIPA (Santa Clara County IPA) with an assigned PCP of one our providers. Please contact our medical office for all insurance related questions.
We do not accept the following insurances:
Anthem Blue Cross Covered CA
Anthem Blue Cross Select
VHP Covered CA
Refills should be called in by your pharmacy and we ask that you please give us 24-48 hours for routine prescriptions. Simply have your pharmacy fax us at (408) 996-3550. You may call your pharmacy to check the status of your refill. Routine prescription requests are not accepted after 5pm or on weekends.
Patients that need a written prescription for mail order, should speak to their provider's Medical Assistant.
These above services/charges are not billed to Insurance.
Writing of Triplicate's will be a minimal office visit charged to your Insurance if you have a copay that will be collected at the time the Triplicate is given.
